C O N S E N T

to the processing of personal data

 

to VANDERBERG MEDIA SE, with registered office at Dolní brána 679/53, 741 01 Nový Jičín, Company ID No.: 178 60 709, entered in the Commercial Register maintained by the Regional Court in Ostrava, Section C, File No. 86729 (hereinafter referred to only as the “Controller”), which operates the web server http://supa24.com, for this Controller to process the following items of my personal data within the meaning of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation), (hereinafter referred to as the “GDPR”), and the valid and effective legislation of the Czech Republic regulating the protection and handling of personal data: 

  • name and surname
  • date of birth
  • residence address
  • phone number
  • email address

(hereinafter referred to only as “Personal Data”).

I.

Purpose and duration of processing

  1. It is necessary to process Personal Data for the purpose of 
    1. using the server http://supa24.com and the business and services offered and provided via it,
    2. marketing or promotion of the server and the services offered and provided via it,
    3. protection of the rights and legally protected interests of the operator.
  2. Personal Data will be processed by the Controller or the Processor referred to in Article III for a period of 5 years.

II.

Revocability of consent

  1. Consent to the processing of Personal Data for the purposes set out in Article I, paragraph 1 may be withdrawn at any time by sending an email to [email protected] or by sending a letter to VANDERBERG MEDIA SE, Porubská 552/28, Ostrava Poruba, 708 00.
  2. The effects of withdrawal of consent come into effect on the date of delivery of written notice of withdrawal to the Controller. 

III.

Processor

Processing of Personal Data is primarily performed by the Controller. However, the following Processors may also process personal data for the Controller: 

  1. Software provider, network administrator: Railsformers s.r.o.
  2. Software provider and marketing support: Religis s.r.o.
  3. Accountants: Account professional a.s.
    1. I can withdraw the consent I now give at any time,

(hereinafter referred to only as the “Processor”).

IV.

Rights of the data subject

I am aware of the fact that:

  1. I can request information from the Controller or Processor about which Personal Data it processes and the purpose for which it processes such data,
  2. I have the right to access and correct or update my Personal Data processed by the Controller or Processor at any time,
  3. I have the right to have the Personal Data processed by the Controller or Processor erased, unless the Controller or Processor has other legal grounds for doing so,
  4. If I have any doubts about compliance with the obligations relating to the processing of Personal Data, I can contact the Controller or the Office for Personal Data Protection.

I hereby declare that I have understood all of the above and that I freely grant this consent to the Controller, as I can refuse or withdraw this consent without being harmed in any way.
